Our billing
department maintains relationships with the third-party
payers with whom we participate.Many of these plans require
a referral from your primary care physician. It is
important that a copy of your referral is on file with both
the plan and our office to ensure continuity of treatment
and timely payment.

All applicable
co-pays are due at the time of service. Please have your
co-pay ready.
